This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free. Dental Services: Dental exams with digital x-rays and electronic dental records Oral hygiene education Simple extractions Surgical extractions Crowns Bridges Prophylaxis Scaling and root planing Root canals Dentures Partials Fillings (amalgams and composite) Sealants Space maintainers Emergencies Participation in all Medicaid and CHIP ProgramsOur Sliding Fee Program offers reduced prices on office visits and services provided in the clinic to all individuals and families who qualify. The Sliding Fee Program is based on family income and may reduce your bill 20%, 40%, 60%, 80% or more.  You may qualify with or without insurance and you may qualify even though you have Medicare or Medicaid.Hours:2nd and 4th Tuesday of each month, 8:15 am – 4:30 pm.Thursdays, 7:30 am – 6:00 pm.Appointments can be scheduled:Monday-Friday, 7:30 am – 6:00 pm.
